The Relationship between Cognitive and Psychoemotional Disorders in Patients with Chronic Heart Failure

Abstract

Relevance. Cognitive impairment (CI) occurs in 20–80 % of patients with chronic heart failure (CHF), significantly worsening the prognosis, quality of life and increasing the medical and social burden. Despite this, screening for CI is often not routinely performed, leading to low detection rates, although undiagnosed CI is associated with an increased risk of hospitalization and mortality. Psycho-emotional disorders (depression, anxiety), often accompanying CHF, mutually aggravate the course of both pathologies.

Aim — to assess the relationship between cognitive and psychoemotional disorders in patients with chronic heart failure.

Materials and methods. The study included 120 patients with CHF, regardless of left ventricular ejection fraction, over 18 years of age in the compensated stage. All participants underwent a comprehensive examination, which included assessment of exercise tolerance using a 6-minute walk test to determine the functional class of CHF, neuropsychological screening of cognitive functions using the Montreal Cognitive Assessment (MoCA), assessment of anxiety and depression using the Hospital Anxiety and Depression Scale (HADS), and assessment of quality of life using the Kansas City Cardiomyopathy Questionnaire (KCCQ).

Results. Cognitive impairment was detected in 87.5 % of patients. Patients with CI were older, had a lower level of education and more severe functional class (FC) of CHF. They also had higher rates of anxiety and depression and lower quality of life. Correlation analysis showed a connection between low cognitive indicators and older age, low education, severity of anxiety and depression, reduced exercise tolerance and low quality of life. Independent predictors of CHF were: older age, lower level of education, depression and severe FC of CHF.

Conclusion. Cognitive impairment is a highly common and clinically significant condition in patients with CHF, closely associated with age, lower education, severity of CHF, and depression. The results highlight the need for routine screening of cognitive function and depression in this population.
