Психотерапевтические методы коррекции поведенческих (нехимических) аддикций: современное состояние доказательной базы

Аннотация

Обзор посвящен анализу современной доказательной базы психотерапевтических методов коррекции поведенческих (нехимических) аддикций.
Цель обзора — систематизировать данные об эффективности этих методов и оценить устойчивость доказательной базы по отдельным формам. Нозологический статус рассматривается прежде всего в рамках действующей в Российской Федерации Международной классификации
болезней 10-го пересмотра (МКБ-10), в которой из всех поведенческих аддикций самостоятельную рубрику имеет лишь патологическое влечение к азартным играм (F63.0) в разделе «Расстройства привычек и влечений»; изменения МКБ-11 привлекаются как вторичный контекст.
На материале систематических обзоров, метаанализов и сетевых метаанализов (преимущественно 2017–2025 гг.) показано, что наиболее
устойчивые доказательства накоплены для когнитивно-поведенческой терапии при патологическом влечении к азартным играм и при зависимости от видеоигр; для интернет- и смартфон-зависимости доказательная база неоднородна и опирается преимущественно на исследования
невысокого методологического качества. Мотивационное интервью, методы «третьей волны», семейная и групповая психотерапия и дистанционные форматы рассматриваются как перспективные дополнения. Сделан вывод о необходимости стандартизации диагностики, долгосрочных
рандомизированных контролируемых исследований и адаптации психотерапевтических программ для отечественной практики.
