Потенциальная роль антипсихотиков третьего поколения в снижении зависимости у пациентов с биполярным расстройством

Аннотация

Мезолимбическая дофаминергическая система играет ключевую роль в патогенезе психических расстройств (шизофрении, биполярного расстройства (БР) и пр.) и развитии расстройств, обусловленных употреблением психоактивных веществ (РПАВ). Общие генетические маркеры и лежащие в основе заболеваний клеточные процессы, например киндлинг (физиологические и поведенческие изменения, вызванные многократным повторением стимулов малой интенсивности), подтверждают тесную связь между этими нарушениями, которая также выражается в высокой частоте сопутствующих заболеваний. Частичные агонисты дофаминовых рецепторов представляют собой новый класс антипсихотических средств с оригинальными фармакодинамическими свойствами, из которых два (карипразин и арипипразол) были одобрены Управлением по контролю за продуктами и лекарствами США для лечения биполярного расстройства (FDA). В то же время целесообразность применения частичных агонистов/антагонистов дофамина при РПАВ не столь однозначна. Последние исследования показали, что дофаминовые D3-рецепторы играют важнейшую роль в развитии РПАВ и БР. Следовательно, допустимо предположить, что частичные агонисты/антагонисты D2/D3-рецепторов можно считать потенциально эффективным классом препаратов для лечения обоих расстройств. В настоящем описательном обзоре рассмотрены перспективы применения упомянутых лекарственных средств в терапии психических заболеваний, особенно у пациентов с коморбидными расстройствами (БР и РПАВ).
