Potential Role of Third Generation Antipsychotics in Reducing Dependence in Patients with Bipolar Disorder

Abstract

The mesolimbic dopamine system plays a key role in the pathogenesis of psychiatric disorders (schizophrenia, bipolar disorder (BD), etc.) and the development of substance use disorders (SUDs).Common genetic markers and underlying cellular processes, such as kindling, physiological and behavioral changes induced by repeated repetition of low-intensity stimuli, confirm the close relationship between these disorders, which also turns into a high level of comorbidity. Dopamine partial agonists represent a new class of antipsychotics with novel pharmacodynamic properties. Two of them including Cariprazine have been approved by the US Food and Drug Administration (FDA) for the treatment of bipolar disorder. At the same time, the expediency of using partial dopamine agonists/antagonists acting on D2- and D3-receptors, in treatment of SUDs is not so unambiguous. However, recent studies have shown that dopamine D3-receptors play a critical role in the development of SUDs and BD. Therefore, it can be assumed that D2/D3 receptor partial agonists/antagonists can be considered a potentially effective class of drugs for the treatment of both disorders. This descriptive review discusses the prospects for the use of these drugs in the treatment of psychiatric disorders, especially in patients with comorbid disorders (BD and SUD).
