Мультимодальный механизм действия препарата маритупирдин и спектр потенциальных клинических эффектов

Аннотация

Несмотря на широкий арсенал доступных средств, значительная часть пациентов с тревожными расстройствами не достигает ремиссии или плохо переносит стандартную терапию, что обусловливает потребность в новых препаратах с анксиолитическим эффектом с иными механизмами действия. В связи с этим интерес представляет оригинальный отечественный мультитаргетный препарат маритупирдин, зарегистрированный в России для терапии генерализованного тревожного расстройства (ГТР). По фармакологическому профилю он преимущественно является антагонистом к 5-HT7-рецептору (константа ингибирования Ki = 153 пМ) с дополнительным антагонистическим воздействием на серотониновые (5-HT2A, 5-HT2C, 5-HT6), адренергические (α2A, α2B, α2C) и гистаминовые H1-рецепторы. В обзоре рассмотрен вероятный механизм действия препарата через призму его рецепторного профиля и спектр потенциальных клинических эффектов. В экспериментальных моделях и первых фазах клинического изучения препарат обнаружил анксиолитические, антидепрессивные и прокогнитивные свойства при низком риске ряда нежелательных явлений, характерных для селективных ингибиторов обратного захвата серотонина и селективных ингибиторов обратного захвата серотонина и норадреналина, являющихся первой линии в терапии тревожных и депрессивных расстройств. Вместе с тем, за исключением доказанного анксиолитического эффекта при ГТР, эти представления имеют преимущественно теоретический и дискуссионный характер и требуют дальнейшего подтверждения в крупных клинических и прямых сравнительных исследованиях с другими препаратами.
