Генерализованное тревожное расстройство: основы диагностики, терапии и специфические для России проблемы
Аннотация
Настоящая статья представляет собой нарративный обзор литературы, цель которого — анализ проблем выявления и диагностики генерализованного тревожного расстройства (ГТР) в России.
Актуальность. ГТР — заболевание, относящееся к пограничным психическим расстройствам. При этом оно носит хронический характер и в отсутствие адекватной терапии приводит к частому присоединению коморбидных расстройств и выраженной дезадаптации пациента, а также к высокому риску суицидальных попыток.
Метод. Поиск осуществлялся в сети Интернет по следующим ключевым словам: генерализованное тревожное расстройство, диагностика, распространенность, критерии DSM, критерии МКБ.
Выводы. В России отсутствуют данные о распространенности ГТР. Согласно имеющимся данным о заболеваемости по всей группе тревожных расстройств, в России уровень заболеваемости существенно ниже, чем в других странах мира. Из всего спектра тревожных расстройств именно ГТР является наименее понятным для отечественных клиницистов. Пациенты с ГТР часто получают ошибочные диагнозы аффективных расстройств, неврозоподобной шизофрении, расстройств личности. Несвоевременная диагностика и отсутствие адекватной терапии приводят к выраженной социальной дезадаптации пациентов, страдающих ГТР. Для решения этих проблем требуются широкомасштабные организационные и образовательные мероприятия, обеспечивающие возможности для своевременной диагностики ГТР.
