Escitalopram and psychotherapy in the treatment for tension-type headache associated with generalized anxiety and panic disorders

Abstract

Relevance. The high comorbidity of tension-type headache (TTH) with anxiety-depressive and panic disorders significantly worsens the course of the disease. To date, the effectiveness of pharmacotherapy, psychotherapy, and the combined use of medication and psychotherapy for this combined syndrome has been insufficiently studied.

Aim – to conduct a comparative analysis of the effectiveness of pharmacotherapy (escitalopram), combination therapy (escitalopram in combination with psychotherapy) and psychotherapy in patients with tension-type headache comorbid with generalized anxiety and panic disorders.

Materials and Methods. A prospective comparative study included 60 patients with episodic and chronic TTH comorbid with generalized anxiety and panic disorders. Patients were assigned to three equal groups of 20 people each: Group 1 (combination therapy: escitalopram – Selektra 10 mg/day + psychotherapy); Group 2 (pharmacotherapy: escitalopram only – Selektra 10 mg/day); Group 3 (psychotherapy). The effectiveness of treatment was assessed using a visual analogue scale (VAS), the Hospital Anxiety and Depression Scale (HADS), the Spielberger-Khanin Reactive and Trait Anxiety Scale, and the SF-36 quality of life questionnaire at four time points: baseline, 14 days, 1 month, and 3 months after the start of therapy.

Results. Combination therapy demonstrated the best results: complete adherence to treatment, the most rapid clinical response (by day 14), and maximum symptom reduction by the third month of observation. In the pharmacotherapy group, the clinical response occurred after one month of therapy, although three patients (15%) discontinued therapy within the first two weeks due to adverse reactions, and four patients discontinued therapy prematurely after improvement. In the psychotherapy group, complete adherence to therapy was recorded, but regression of anxiety and depressive symptoms was achieved slowly, while the number of panic attacks remained virtually unchanged.

Conclusion. Combination therapy (escitalopram + psychotherapy) is the preferred treatment for patients with TTH associated with comorbid anxiety, depressive, and panic disorders. Pharmacotherapy produces good results but is associated with the risk of treatment failure and a longer period before clinical benefit is achieved. Psychotherapy has high adherence rates but requires additional financial costs and modifications to therapy for patients with panic attacks.
