Metabolic Consequences of Antipsychotic Therapy: Psychiatrists’ Opinion

Abstract

Relevance. The American Psychiatric Association (APA), in collaboration with other medical societies, developed recommendations for the monitoring of metabolic syndrome in patients taking antipsychotics back in 2004. Despite these simple recommendations and recognition by psychiatrists of the importance of monitoring metabolic disorders, a number of studies indicate non-compliance with these recommendations by physicians prescribing antipsychotics.

The aim — to study the level of awareness, attitudes and compliance of psychiatrists with the basic recommendations for monitoring metabolic disorders in patients taking antipsychotics, as well as the perceived barriers to its implementation.

Material and methods. An anonymous survey of 70 psychiatrists in Kemerovo and Tomsk was conducted using the questionnaire “Primary Care Providers’ Views on Metabolic Monitoring of Outpatients Taking Antipsychotic Medication”, developed for primary care physicians in order to assess their attitudes toward barriers to monitoring metabolic disorders in patients and characterize their beliefs about responsibility for conducting such monitoring.

Results. The majority (97.1%) of physicians were well aware of the metabolic risks associated with antipsychotic drugs. At the same time, 70% of physicians agreed with the statement that individuals with schizophrenia are at higher risk of developing metabolic disorders than the rest of the population. Although many (40.1%) believe that monitoring risk factors for metabolic disorders is the responsibility of primary care physicians, most psychiatrists surveyed still agreed that this task is part of their responsibilities. About half (51.4%) of respondents disagreed with the statement that it is the responsibility of a psychiatrist to prescribe medications to treat elements of metabolic dysfunction. Among all possible obstacles, the greatest impact on monitoring, according to psychiatrists, is the lack of medications in their arsenal for the correction of metabolic disorders — 18.6%.

Conclusion. A survey of psychiatrists regarding the monitoring of metabolic disorders in patients taking antipsychotics indicates a high degree of awareness of the side effects of antipsychotic therapy and a willingness to cooperate in screening. However, the main obstacle on this path, according to psychiatrists, is the lack of drugs in their arsenal to correct metabolic disorders.
