Negative symptoms in schizophrenia: assessment approaches

Abstract

The review article presents modern approaches to the systematics and diagnosis of negative symptoms in schizophrenia. Special attention is paid to the differentiation of primary and secondary negative symptoms, as well as the specification of negative symptoms detected during exacerbation of psychotic symptoms and with a predominance of negative symptoms. As a separate issue, criteria for the effectiveness of therapy for predominantly negative symptoms and quantitative indicators of minimal clinical improvement are considered. An analysis of the results of two studies conducted on the pooled database of studies of cariprazine in patients with schizophrenia with persitant negative symptoms is presented.Assessment of the predominantly negative symptoms is preferable to use the domain of negative symptoms of the PANSS scale, with the possibility of supplementing them with the BNSS and CAINS scales. Minimal clinical improvement in patients with predominantly negative symptoms is recorded with a reduction in the total score on the subscale of negative symptoms in the range from 7.3 to 18.5%.
