Abstract

This article presents scientific views on the formation and clinical dynamics of post-traumatic stress disorder (PTSD) developing during combat mental injuries, as well as current data on its prevalence and diagnostic criteria. The results of our own research are presented, in which 84 servicemen with acute psychogenic disorders developed in combat conditions were examined. Mental disorders in this group of patients most often referred to the neurotic level of mental pathology. The following clinical variants of acute psychogenic disorders in combat mental trauma were identified: with a predominance of behavioral disorders – 57.0%; neurotic symptoms – 41.7 %; psychotic disorders – 1.3 %. A mathematical model for predicting the long-term consequences of acute stress disorders and adaptation disorders and acute stress disorders in the form of PTSD using a number of logical and statistical methods is described. Statistically significant (p < 0.001) clinical variants of the acute period and remote stages of combat mental trauma are described in detail, allowing predicting favorable and unfavorable outcomes. The clinical symptoms of the acute period of combat trauma are identified, which are predictors of an unfavorable outcome, namely: a high level of personal anxiety, affective tension, impulsivity, depressive symptoms with a predominance of apathy. The importance of subjective assessment of patients› condition for the prognosis of long-term consequences of combat mental trauma and the formation of prolonged PTSD is shown.
