Исходы стационарного лечения посттравматического стрессового расстройства у комбатантов

Аннотация

Методология. 161 комбатант, участник контртеррористической операции на Северном Кавказе, из них 108 с диагнозом посттравматического стрессового расстройства (ПТСР), были обследованы во время госпитализации в психиатрические отделения. При обследовании были использованы клинико-психопатологический метод, шкала профиля ПТСР, шкала глобальной оценки функционирования и шкала социальной адаптации.

Результаты. Структура психических расстройств у обследованных комбатантов определяется преобладанием невротических и связанных со стрессом расстройств (73,3 %), среди которых ПТСР составляло 67,1 %. Различия в симптоматическом профиле позволило выделить три основных клинических варианта боевого ПТСР: тревожно-эксплозивный (51,9 %), диссоциативный (32,4 %) и апатичный (15,7 %). Первый являлся прогностически благоприятным (выздоровление в 37,5 %), в то время как при двух других положительный исход составил только 17,2 и 17,7 % соответственно. Наиболее важными факторами в прогнозе ПТСР были клинический вариант расстройства (Коэффициент корреляции (КК) = –0,22), выраженность дисстрессирующих реминисценций (КК = –0,30) и наличие черепно-мозговой травмы в анамнезе (КК = –0,24).

Выводы. Результаты исследования могут быть полезны при проведении экспертизы годности к военной службе.
