Abstract

Background: Tobacco smoking is a leading preventable cause of poor health and premature mortality. While tobacco smoking rates in the general population are declining, they remain raised in cohorts with psychiatric disorders. This narrative review highlights the epidemiology, consequences and treatment options of tobacco use disorder (TUD) and nicotine dependence (ND) in people with bipolar disorder (BD).

Methods: This article is based upon a Medline literature search from 1970-2021 using MeSH terms “Bipolar Disorder”” x “Smoking” or “Nicotine” or “tobacco”. Search results were complemented by additional literature retrieved from examining cross references and by hand search in text books.

Summary of findings: Lifetime and point prevalence of smoking in people with BD is in the range of 45-70% and approximately 2-3 times more prevalent in BD than in the general population. TUD and ND have a detrimental impact both on mental and physical health in people with BD. In the absence of large controlled studies in comorbid BD and TUD or ND, pharmacological treatment adheres to the individual guidance for each disorder. Psychosocial interventions delivered by community mental health services and established for use in TUD and ND appear to be applicable in people with BD, too, as well as Cognitive Behavioural (CBT) or Acceptance and Commitment (ACT) based psychotherapy.

Conclusions: Smoking is a modifiable life style factor that puts people with BD on increased risks both for mental and physical health, and deserves special attention in treatment. However, more treatment research is needed to characterize optimally tailored pharmacological and psychosocial interventions in comorbid BD and TUD or ND.
