Pathokinesis of endogenous depressive-hypochondriacal disorders of a non-psychotic register in therapy by sulpiride

Abstract

Objective: to study clinical features and dynamics of symptoms in therapy by sulpiride in different variants of non-psychotic types of an endogenous depressive –hypochondriacal syndrome. Material and methods: 47 patients (36 women and 11 men) with a depressive episode (n = 15), a recurrent depressive disorder – DD (n = 14) and a slowly progressive schizophrenia SPS (n = 18) were examined clinically and with usage of psychometric scales: the Clinical Global Impression Scale-CGI, Montgomery Asberg Rating Scale (MADRS), Hamilton Anxiety Rating Scale (HARS) and Side-Effect Rating Scale (UKU). Sulpiride was indicated in initial daily dose of 50–100 mg, if required, the daily dose was increased up to 400–600 mg. Results: After 8 weeks of therapy in patients with affective disorders the MADRS and HARS scores showed reduction from 28,7 ± 2,3 to 14,3±1,7, and from 14.8±2.1 to 7,4-2,7 respectively. The reductions of depressive symptoms and anxiety were 50,2% and 50,0%, respectively. In SPS the mean MADRS and HARS scores decreased from 21,4+_3,7 to 13,7±1,8, and from 10,2±1,5 to 6,4±3,2 respectively. Symptoms of depression decreased totally by 40%, anxiety – by 37,3%. Conclusion: In patients with affective disorders the efficacy of Sulpiride in depressive - hypochondriacal syndrome is predominantly due to its antidepressive and anti-anxiety effects, and to its antipsychotic and activating effects in SPS.
