Когнитивная уязвимость у пожилых пациентов c депрессией: кто в группе риска?

Аннотация

Актуальность. Депрессия у пожилых людей является одной из основных причин глобальной инвалидности и ухудшения качества жизни. Она часто сопровождается когнитивными нарушениями, которые могут сохраняться и после завершения депрессивной фазы. Выявление клинико-биологических факторов, влияющих на развитие и прогрессирование когнитивных расстройств, является важной задачей современных исследований.

Цель исследования — изучение влияния клинико-биологических характеристик больных с поздними депрессиями на их когнитивное функционирование (клинико-катамнестическое исследование).

Материал и методы. В исследовании приняли участие 51 пациент в возрасте 60 лет и старше, страдающий депрессиями позднего возраста и проходивший лечение в геронтопсихиатрическом стационаре. Повторное обследование проводилось через 1 и 3 года. По итогам стационарного лечения пациенты были разделены на две группы: 1-я группа — 38 человек (74,5 %) без когнитивных нарушений в структуре депрессии и 2-я группа — 13 человек (25,5 %) с жалобами на ухудшение памяти. Далее проводилось сравнительное проспективное катамнестическое наблюдение за пациентами этих групп. Обследование включало клинико-психопатологический, психометрический, клинико-катамнестический, иммунологический и статистический методы.

Результаты. Пациенты с когнитивными нарушениями (2-я группа) имели более старший возраст на момент обследования (73,4 ± 7,7 лет) по сравнению с пациентами 1-й группы (67,8 ± 5,9 лет, p < 0,05). Также у них чаще встречались депрессии из круга негативной аффективности (76,9 % против 55,3 %, p < 0,05), поликоморбидная соматическая патология, включая кардиальные заболевания (100 % против 76,3 %, p < 0,05), и терапия была менее эффективной (p < 0,001). Иммунологические характеристики также различались: все пациенты с жалобами на ухудшение памяти относились к иммунофенотипу 2, характеризующемуся пониженной активностью лейкоцитарной эластазы и ее протеазного ингибитора.

Заключение. Исследование показало высокий риск ухудшения когнитивного функционирования у пожилых пациентов с депрессией, связанный с определенными клинико-биологическими факторами. Среди предикторов когнитивного снижения были выявлены: более старший возраст, преобладание негативной аффективности в структуре депрессии, высокая отягощенность поликоморбидной соматической патологией, неполные ремиссии и особенности иммунного статуса.
