Использование антагонистов опиоидной системы в терапии пациентов с пограничным расстройством личности: связь механизмов действия с воспалением и метаболическими эффектами
Аннотация
В настоящее время использование основных психофармакологических групп при лечении пограничного расстройства личности (ПРЛ) не является доказательно эффективным. Существующая гипотеза о нарушениях функционирования эндогенной опиоидной системы при ПРЛ привела исследователей к выводам, что антагонисты опиоидов могут купировать некоторые симптомы этого заболевания. В данном обзоре проведен анализ доступной литературы, в которой исследовалось влияние антагонистов опиоидов на течение ПРЛ. Поиск находящихся в открытом доступе опубликованных работ осуществлялся на русском и английском языках. Запросы включали названия зарегистрированных на данный момент лекарственных средств, являющихся полными или частичными антагонистами опиоидных рецепторов. Было обнаружено 10 соответствующих критериям поиска работ, которые представляли собой плацебо-контролируемые, в том числе рандомизированные (n = 2), продольные (n = 3) и ретроспективные исследования (n = 1), а также описания клинических случаев (n = 4). В большинстве работ применялся налтрексон (n = 6). В единственном исследовании, где применялся налоксон, не было обнаружено положительного эффекта при симптомах ПРЛ. В остальных работах полученные данные демонстрируют, что назначение препаратов из этой группы может быть эффективным в терапии ряда симптомов, таких как несуицидальное самоповреждающее поведение, импульсивность, суицидальные тенденции и диссоциативные симптомы. В статье также обсуждаются возможные влияния нарушений опиоидной системы на другие эндокринные системы, вызванные метаболические эффекты и системное воспаление в рамках данного расстройства. Также результаты работы позволяют предположить, что одну из ключевых ролей в нарушениях при этом расстройстве играет не только эндогенная опиоидная система в целом, но и по большей части доступность динорфина или каппа-опиоидных рецепторов, от которых зависит активность дофаминергических нейронов, оказывающих влияние на гедонический порог и механизмы положительного и отрицательного подкрепления, что может служить объяснением многих симптомов пограничного расстройства. Дальнейшее изучение указанного вопроса может быть перспективным в формировании новых патогенетически обоснованных подходов к терапии ПРЛ.
