Наблюдательное исследование применения психофармакотерапии у пациентов с нервной анорексией с экстремально низким весом при лечении в стационаре

Аннотация

Актуальность. Доказательная база в отношении тактики лечения больных нервной анорексией (НА) с экстремально низким весом на сегодняшний день отсутствует, тем не менее такие пациенты обращаются в психиатрические учреждения за медицинской помощью. Существует потребность в накоплении данных по подходам к лечению таких пациентов.

Цель — проанализировать наиболее частые назначения психофармакотерапии у больных НА с экстремально низким индексом массы тела (ИМТ < 13 кг/м2) при лечении в круглосуточном стационаре и оценить результаты лечения на момент выписки из стационара и через 1–2 года после выписки.

Методы. Ретроспективный анализ историй болезни (n = 47) с оценкой состояния пациентов в катамнезе (1–2 года). Результаты лечения оценивали по следующим параметрам: ИМТ при выписке и в катамнезе, занятость, качество жизни, удовлетворенность лечением, смертность. Качество жизни оценивали при катамнестическом обследовании с помощью опросника качества жизни Всемирной организации здравоохранени краткая версия (ВОЗКЖ-26). Удовлетворенность лечением оценивали с помощью опросника «Удовлетворенность стационарным лечением».

Результаты. Все пациенты получали комплексную терапию, из антипсихотиков наиболее часто применяли оланзапин (54,5 %), из антидепрессантов — миртазапин (57,1 %), гидроксизин применяли у 55,3 % пациентов. При выписке ИМТ составило более 16 кг/м2 у 63,8 % пациенто при катамнестическом наблюдении — у 72,7 % . Все пациенты были живы на момент катамнестического обследования, 88,9 % работали или учились. Индекс массы тела при выписке отрицательно коррелировал с возрастом пациентов (r = –0,29, p < 0,05) и продолжительностью болезни (r = –0,31, p < 0,05), при катамнестическом наблюдении этих корреляций не отмечалось. Результаты, полученные с помощью ВОЗКЖ-2 превышали популяционные нормы, удовлетворенность лечением в стационаре была более 70 %.

Выводы. Оланзапин, миртазапин и гидроксизин являются наиболее часто назначаемыми препаратами при терапии больных НА с экстремально низким весом. Существует необходимость рандомизированных контролируемых исследований по применению психофармакотерапии при НА.
