Поддерживающая терапия с помощью ритмической транскраниальной магнитной стимуляции у пациентов с депрессивным эпизодом: обзор литературных данных
Аннотация
В клинической практике для поддержания клинического эффекта и профилактики рецидивов у пациентов с рекуррентной депрессией повсеместно используется медикаментозная терапия, однако ее возможности у пациентов с фармакорезистентными эпизодами ограничены. В обзоре рассмотрены имеющиеся данные об эффективности применения ритмической транскраниальной магнитной стимуляции (рТМС) в качестве поддерживающей терапии. В частности, обобщены такие важнейшие методологические аспекты, как выбор когорт пациентов, длительность поддерживающей терапии и временной паттерн поддерживающих сессий, параметры протокола (мишень, частота, тип стимуляции и др.), возможности сочетания с медикаментозной терапией. В большинстве опубликованных исследований продемонстрирована эффективность поддерживающей терапии рТМС, но высокая гетерогенность протоколов стимуляции, а также малое число рандомизированных клинических исследований не позволяют рекомендовать какой-то конкретный алгоритм для применения в широкой клинической практике. Тем не менее поддерживающая терапия рТМС может не иметь альтернативы для определенных групп пациентов, что обусловливает необходимость проведения крупных рандомизированных контролируемых исследований для определения оптимальных для применения в клинической практике характеристик протокола стимуляции и ее длительности с оценкой не только клинической, но и экономической эффективности.
