Динамика когнитивных нарушений у больных с депрессивными расстройствами в процессе комбинированной и монотерапии антидепрессантами

Аннотация

Цель исследования. Сравнить динамику когнитивных нарушений у больных с депрессивными расстройствами в процессе комбинированной и монотерапии антидепрессантами.

Материал и методы. Включены 198 пациентов с депрессивными расстройствами и 131 здоровый доброволец, проходивших психодиагностическое тестирование когнитивных функций на программно-аппаратном комплексе «Мультипсихометр-05», включавшее оценку памяти, внимания, скорости реакции, исполнительных функций и мышления. Оценка выраженности аффективной симптоматики у пациентов с депрессивными расстройствами проводилась с помощью шкалы депрессии MADRS.

Результаты. Терапия антидепрессантами оказывает положительное влияние на редукцию когнитивных нарушений у пациентов с монополярными депрессивными расстройствами. Применение нейрометаболических/нейропротекторных препаратов в комбинации с антидепрессантами имело объективное обоснование в виде выраженных снижений памяти и способности к сосредоточению, однако положительное действие на когнитивные дисфункции комбинации нейрометаболических/нейропротекторных и тимоаналептических препаратов в среднем оказалось сопоставимым с монотерапией антидепрессантами. В случае клинической необходимости краткосрочное назначение транквилизаторов/гипнотиков может быть рекомендовано пациентам с непсихотическими депрессивными расстройствами без существенного риска негативного влияния на познавательные процессы.
